Submit a Complaint

Complaints and compliance concerns received by the Utah Bureau of Emergency Medical Services (BEMS) are forwarded to the Compliance Officer or authorized designee for review in accordance with Utah Code § 53-2d-606 and Utah Administrative Rule R911-5-1202. Complaints are reviewed to determine whether credible information exists to initiate a formal investigation.

  • Complaints may involve EMS personnel, agencies, educators, training officers, course coordinators, or training programs. Where appropriate, BEMS may coordinate with or refer matters to internal programs, including the EMS Specialty Care and Education Program, for subject-matter review or agency-level oversight. Referral does not preclude BEMS from initiating a formal investigation when warranted.
  • BEMS initiates formal investigations when credible information indicates a potential violation of statute, administrative rule, or professional standard. All complaints are reviewed, documented, and addressed based on the nature of the concern.
Please Note: Based on the outcome of a review or investigation, BEMS may recommend enforcement actions as authorized by law, including letters of notice, corrective action, fines, denial, probation, suspension, or revocation of a license, designation, or endorsement.

BEMS encourages individuals to report concerns in good faith. Complaints may be submitted anonymously or by confidential sources. BEMS will protect the identity of complainants to the extent permitted by law.
